Disclaimer

In checking the box below, I hereby agree that I am choosing to participate in breathwork sessions, classes, and/or other services offered by Jill Aldridge, Restorative Somatics, and Jill Aldridge, LLC. For the purposes of this Release and Waiver, the term “Releasees” shall refer to any or all of:
 
1. Jill Aldridge
2. Restorative Somatics and Jill Aldridge, LLC
3. The heirs, assigns, successors, employees, volunteers and/or associates of Jill Aldridge and/or Restorative Somatics and Jill Aldridge LLC

I understand and acknowledge that in the discussions, consultations, and breathwork session(s) I have, the Releasees:

a) are not intended to replace any relationship I have with my medical doctor and/or primary health care provider(s)
b) are not intended to constitute medical advice or any substitution for medical care
c) are not intended to be relied on for prescriptions, recommendations, diagnosis or treatment in relation to any health problem or disease

I understand that if I am taking any medications or have any medical conditions such as, but not being limited to: cardiovascular disease, detached retina or glaucoma, diagnosed brain or abdomen aneurysm, epilepsy, diagnosed schizophrenia or bi-polar, uncontrolled diabetes or thyroid conditions, high blood pressure, or pregnancy, that I must advise Jill Aldridge or the facilitator/s.

I certify that I have taken medical advice relating to any physical, mental or emotional condition that may impair my judgment, or have any affect on my physical health in any way during or after a session of breathwork.

I understand and acknowledge that I am responsible for consulting my primary health care provider or a medical doctor in the event that I have or am suspect to have a health problem.

I am encouraged by the Releasees to make my health care decisions in partnership with my medical doctor and/or primary health care provider(s) on the basis of my own research regarding the effectiveness of breathwork sessions and the importance of diet, exercise, supplementation, stress management and emotional and mental work.

I understand and acknowledge that, in undertaking a breathwork session with any of the Releasees, I am doing so at my own risk. It is with that understanding that I voluntarily execute this release and waiver. Since breathwork is experiential and the extent of the risks and benefits are not fully known, I agree to assume and accept full complete responsibility for any known and unknown risks associated with my participation in breathwork, including any physical injury, psychological or emotional effects, death, loss, or property damage.

I take full responsibility for my physical and emotional well-being while attending classes, courses, workshops, or other services with Jill Aldridge, LLC. I agree to pace myself properly or stop breathwork practice altogether if need be.

Audio/Video Release 
Jill Aldridge will be recording classes, courses, workshops and your voice/image may be captured. By participating in classes, courses, workshops you consent to these audio/video recordings and agree that they may be edited, duplicated, distributed, reproduced, or reformatted in any manner including copyright without payment of additional fees, in perpetuity. If you prefer not to be captured, please mute your audio and/or turn off your video.

I agree to not record by audio, video, photographic or any other means, any portion of classes, courses, workshops, or other services.

I understand that I take full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in sessions, classes, courses, or workshops. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Jill Aldridge, LLC or its members.
